  1. General provisions<\/strong>\n
      \n
    1. Gift vouchers are issued by DR KOBERDA CLINIC, with its registered office in Gdynia, ul. Red\u0142owska 52\/1.<\/li>\n
    2. The voucher is a document entitling the bearer to use the services provided at the clinic in accordance with the terms and conditions set out in these regulations.<\/li>\n
    3. The voucher is not a means of payment and cannot be exchanged for cash.<\/li>\n<\/ol>\n<\/li>\n
    4. Purchase and fulfillment<\/strong>\n
        \n
      1. Vouchers are available for purchase only at the clinic.<\/li>\n
      2. The voucher can be issued for:\n
          \n
        1. \u2022 a specific amount to be used for any services,<\/li>\n
        2. \u2022 a selected dental treatment from the clinic’s offer.<\/li>\n<\/ol>\n<\/li>\n
        3. The voucher can be used once or in parts until its value is exhausted.<\/li>\n
        4. If you choose a voucher with a specific amount, and the value of the service exceeds the value of the voucher, the difference must be paid.<\/li>\n<\/ol>\n<\/li>\n
        5. Validity of the voucher<\/strong>\n
            \n
          1. The voucher is valid for 12 months from the date of purchase (the expiration date is indicated on the voucher).<\/li>\n
          2. Failure to use the voucher within its validity period will result in the loss of the right to redeem it, without the possibility of a refund.<\/li>\n<\/ol>\n<\/li>\n
          3. Terms of use<\/strong>\n
              \n
            1. The voucher must be presented at the clinic reception desk before the appointment begins.<\/li>\n
            2. The voucher can only be redeemed at the DR KOBERDA clinic.<\/li>\n
            3. The voucher cannot be exchanged for cash or refunded.<\/li>\n
            4. It is possible to add a personal dedication to the voucher.<\/li>\n
            5. The voucher may be transferred to a third party (it can be redeemed by any bearer).<\/li>\n<\/ol>\n<\/li>\n
            6. Complaints and returns<\/strong>\n
                \n
              1. Complaints related to the purchase or redemption of vouchers can be submitted in writing or by email to drkoberda@gmail.com.<\/li>\n
              2. Vouchers are non-refundable and cannot be exchanged for cash.<\/li>\n<\/ol>\n<\/li>\n
              3. Final provisions<\/strong>\n
                  \n
                1. The purchase of a voucher is tantamount to acceptance of these terms and conditions.<\/li>\n
                2. The clinic reserves the right to make changes to the regulations.<\/li>\n
