Terms and conditions of use of the voucher

General provisions
1. Gift vouchers are issued by DR KOBERDA CLINIC, with its registered office in Gdynia, ul. Redłowska 52/1.
2. The voucher is a document entitling the bearer to use the services provided at the clinic in accordance with the terms and conditions set out in these regulations.
3. The voucher is not a means of payment and cannot be exchanged for cash.
Purchase and fulfillment
1. Vouchers are available for purchase only at the clinic.
2. The voucher can be issued for:
  1. • a specific amount to be used for any services,
  2. • a selected dental treatment from the clinic’s offer.
3. The voucher can be used once or in parts until its value is exhausted.
4. If you choose a voucher with a specific amount, and the value of the service exceeds the value of the voucher, the difference must be paid.
Validity of the voucher
1. The voucher is valid for 12 months from the date of purchase (the expiration date is indicated on the voucher).
2. Failure to use the voucher within its validity period will result in the loss of the right to redeem it, without the possibility of a refund.
Terms of use
1. The voucher must be presented at the clinic reception desk before the appointment begins.
2. The voucher can only be redeemed at the DR KOBERDA clinic.
3. The voucher cannot be exchanged for cash or refunded.
4. It is possible to add a personal dedication to the voucher.
5. The voucher may be transferred to a third party (it can be redeemed by any bearer).
Complaints and returns
1. Complaints related to the purchase or redemption of vouchers can be submitted in writing or by email to drkoberda@gmail.com.
2. Vouchers are non-refundable and cannot be exchanged for cash.
Final provisions
1. The purchase of a voucher is tantamount to acceptance of these terms and conditions.
2. The clinic reserves the right to make changes to the regulations.
3. The regulations are available at the clinic’s headquarters and on the website drkoberda.pl.
